Care Coordination & Disease Management Support
COORDINATED CARE
RelateCare provides individuals with proactive outreach, education, and clinical resources to enable them to continuously support their health.
RelateCare’s Chronic Care Coordination & Disease Management Support programs provide proactive, patient-centered care to individuals managing long-term health conditions or recovering from a hospital stay. Our approach combines clinical expertise with digital health solutions to enhance patient engagement, improve self-management, and reduce hospital readmissions.
Through our Care Coordination, Chronic Condition Management and Hospital/Surgical Admission and Discharge Programs, RelateCare nurses engage patients to guide them in management of their conditions as well as support preparation for and recovery from hospital admissions. Our customizable programs can include disease/condition specific management as well as episodic approaches to supporting patients including night before surgery communication and post discharge follow-up. Our teams have demonstrated results in readmission reduction, HCAHPS score improvement, and patient adherence to clinical guidelines.
Patient Centred
Care coordination requires a comprehensive, patient-centered approach that extends beyond traditional healthcare settings.
By delivering proactive, technology-enabled care, RelateCare ensures that patients living with chronic conditions or who are scheduled for procedures or admissions receive the guidance and support necessary to enhance their quality of life and reduce unnecessary hospitalizations.
