Post Discharge Follow Up Program RelateCare.

What is it?

The Post Discharge Follow Up Program is a program aimed at reaching out to patients after they have been discharged from hospital in order to guide them through their transition home, reassure them about their recovery process, remind them of their medication adherence guidelines, and catch any red flags.

How does it work?

Within 24 hours of discharge from the hospital, patients receive a call from a Registered or Student Nurse. The call provides the opportunity for the patient to discuss any issues they may be having with the transition home and allows the healthcare provider to identify any risks or “red flags” that may eventually lead to readmission.

Why do you need it?

The Affordable Care Act has made it a necessity for hospitals to reduce unnecessary readmission, or else suffer financial penalties. Following up with patients after discharge is proven to reduce readmissions, and it also increases patient satisfaction; both valuable indicators of a strong patient-provider relationship.
